Comprehensive Primary Care
Michael Thorarinson, DNP, RN, FNP-BC received his Bachelor of Science in Nursing at the University of North Carolina at Greensboro and his Masters of Science in Nursing as a Family Nurse Practitioner at the University of North Carolina at Chapel Hill. He received his Doctorate in Nursing Practice at Gardner-Webb University.

Fees
Complete Primary Care fees are set at the usual and customary rates for this area. Fees are also determined by the complexity of the problem you may have. If you have questions about our fees please feel free to ask. We will be glad to answer your questions.
Insurance
If you have insurance coverage, please let us know before you are seen. If your insurance changes, please inform the staff when you check in. You must bring your insurance card to each visit! Your insurance policy is a contract between you and your insurance company. Insurance is verified prior to each visit to ensure coverage and benefits. Co-payments, deductibles and other balances not covered by your insurance carrier are your responsibility. Upon check out, we will estimate your deductible and/or co-payments so that you can pay at that time. You are encouraged to discuss any questions regarding our fees with us.
Sliding Fee Discount Patient Application
Complete Primary Care has a deep commitment to our community and the patients we serve. Likewise, The National Health Service Corps (NHSC) builds healthy communities by supporting qualified health care providers dedicated to working in areas of the United States with limited access to care. The NHSC approves sites that provide outpatient, primary health services to areas that are experiencing health professional shortages. Complete Primary Care’s Sliding Fee Schedule Policy is in alignment with these core values of NHSC.
An application can be obtained at the office front desk or calling the office. Applications can be returned by the patient, fax or mail.

Please complete the following information and return to our office. This application will be used to determine if you or members of your family are eligible for a discount.
This discount will apply to office visits and office based services. This does not include services from contracted reference labs, diagnostics performed outside of the office or prescriptions/immunizations.
We will work with you to coordinate community services/resources for your prescription or vaccination needs. You may apply for additional discounted services with an NHSC recognition.
